Letter of Support Request Form

This form is designed to assist nonprofits, organizations, or government agencies in obtaining a letter of support from Assemblymember Dr. Darshanna Patel for grant applications. This process is not used for letters for individuals.

To request a letter of recommendation, please complete the form below. We require at least 10 business days to process a request for a letter of support for grant applications. This allows us ample time to complete it. You can also upload materials you want to include for reference.

All requests for letters of support will be reviewed by the Assemblymember’s office and are subject to approval. Approved drafts will be edited accordingly.
