AANHPI Heritage Month Leadership and Service Recognition Nominations

Eligibility 
Any Senate District 33, Asian-American, Native Hawaiian and Pacific Islander founded organization, individual, or group of volunteers that has demonstrated commitment and dedication to Senate District 33 through significant and direct volunteer service can be nominated. 

Nominate
The nominator must describe the organization or individual's volunteer service and accomplishments by answering the questions in this nomination form. 

The deadline for nominations is April 19, 2024, by 5:00 p.m.

Nominee (Organization or Individual First and Last Name): *

- Provide a brief description of the organization/individual's leadership and service in the community. *

- What were outcomes of the nominee’s volunteerism? Provide as much detail as possible, for example: number of volunteer hours provided, number of people served, etc. *

- What made this nominee’s volunteer service extraordinary? Provide anecdotal information, such as stories and quotes to demonstrate the non-measurable impact of the nominee’s volunteerism. *

- Provide social media account handles, online biography links, or any other useful online links that could provide more information about this nominee's accomplishments.  *
